School Administration
Please click on the links below to print out each document. Completed forms are due by Friday, August 15 to the School Office. Please contact the office directly at 262-367-2040 or by email at school@stcharleshartland.org with any questions.
- Archdiocesan Student Acceptable Use Policy (required for each student)
- Archdiocesan General Consent Form for Photo, Video, and Audio Use (required for each family)
- Department of Health Services Student Immunization Information and Record Form (required for each student)
- Archdiocesan Confidential Family/Student Information Form (only for families with divorced, separated, widowed, remarried, or unmarried parents/guardians)
- Peanut/Tree Nut-Free Zone Information
- Medication Forms: If your child uses an inhaler, epi-pen, or glucagon, please complete the authorization forms. If your child uses prescription or over-the-counter medications that you would like to store in the Health Room, please complete the prescription medication and/or over-the-counter medication form. As a reminder, parents must provide these medications to the School Office.
- Home & School Association Forms: Please read a letter from Home & School Association President Tera Denk. For more information about the SCRIP program and to pledge how you will support the HSA's fundraising efforts through SCRIP, please print and complete the SCRIP forms.
